Modu-Laser Product Service
Modu-Laser's service department provides complete warranty and non-warranty service for all Modu-Laser products.  If service to your Modu-Laser product becomes necessary, please follow the steps below:







1.   Contact Modu-Laser at (801) 983-2682 and obtain a Return Authoriztion number.
2.   Download the
3.   Print the "RA Label"
4.   Fill in the required information on the "RA Label"
5.   Carefully package the device for shipment (using original box and foam if possible) to prevent damage during shipment.
6.   Securely affix the "RA Label" to the shipping container.
7.   Use a reputable carrier (FEDEX or UPS suggested) to send the device to the address below:

Modu-Laser will not be liable for any loss or damage to any device which may occur during the shipment of said device to Modu-Laser. It is recommended that any device being sent to Modu-Laser be appropriately insured prior to shipment. It is also recommended that a reputable carrier, such as United Parcel Service (UPS) or Federal Express (FEDEX), be used for any shipment to Modu-Laser. If it is determined, after receiving and evaluating a Modu-Laser product, that warranty service is allowable, Modu-Laser will cover the cost of repair (or replacement) of any defective device component as well as the cost of return postage. For service of non-Modu-Laser products, the customer is responsible for all repair and shipping costs.
