Modu-Laser's service department provides complete warranty and
non-warranty service for all Modu-Laser products.
If service to your Modu-Laser product becomes necessary, please follow
the steps below:

1. Contact Modu-Laser at (801)
983-2682 and obtain an Return Authorization (RA) number.
(Note: Be prepared to provide the
date of purchase,
device serial number, and your contact information.) |

2. Download the PDF "RA"
label by

3.
Print the "RA" label |
|
4.
Use an indelible marker to
fill-in the required information on the "RA"
label. |
|
5.
Carefully package the device
for shipment (using original box and foam if possible) in
such a manner as to prevent damage
during transit. |

6.
Securely affix the "RA"
label to the
shipping container. |
|
7.
Use a reputable carrier
(FEDEX, UPS or DHL preferred) to send the device to the
address below: |

Modu-Laser will not be liable for damages incurred
during shipment of the device. Modu-Laser recommends
that the purchaser insure the unit during shipment for a
reasonable value (purchase price or current estimated
value). Modu-Laser also recommends that the product be
returned via United Parcel Service (UPS) or Federal
Express (FEDEX). If it is determined, after receiving
and evaluating the device, that warranty service is
allowable; Modu-Laser will cover the cost of repair (or
replacement) of any defective unit component as well as
the cost of return postage. If it is determined
that the unit requires service that is not covered by
the provisions of the device warranty, Modu-Laser will
contact the purchaser for authorization prior to
performing any service on the device. Purchaser will
cover cost of repair and return postage charges for all
units serviced outside of the provisions of the device
warranty.
